Reverse Pull Headgear, also known as a Delaire-type facemask, is an orthodontic appliance designed to assist in the treatment of maxillary deficiency and Class III malocclusion in growing children. It works by applying forward-directed forces to help encourage the upper jaw (maxilla) to move forward and improve its relationship with the lower jaw. This type of growth modification treatment is commonly considered when the upper jaw is small or positioned behind the lower jaw, often resulting in an anterior crossbite where the upper front teeth sit behind the lower teeth. Treatment may be started during the growing years, particularly after the permanent six-year molars have erupted, and is generally most effective during active growth. Reverse Pull Headgear treatment may also be used together with palatal expansion to widen the upper jaw and help create a more ideal bite. The slightly downward direction of the elastics may also assist in improving the jaw relationship by encouraging the lower jaw to grow downward and backward.
